Sacred Leadership Global Yoga Liability Release Form

I do hereby give my permission for my participation in yoga classes taught by Allison Adams (virtually pre-recorded, live online, and/or in-person) through Sacred Leadership Global LLC. 

By signing this waiver, I acknowledge this is a physically intense yoga class, I am in good health and able to participate. I have visited a licensed physician within the past 6 months and have been cleared to practice strength-based yoga. I acknowledge it is my responsibility to modify any yoga postures, breath work, or meditation to my needs, at any and all times. This includes taking responsibility and care for any pre-existing medical conditions or injuries that may impact my practice.

In consideration of the permission granted to me to participate in yoga classes, I do hereby agree, on my own behalf, to release Allison Adams and administrators, from any and all actions, causes of action, damages, claims, or demands of whatever kind of nature which I may have for injuries, known or unknown, which are incurred by, arise from, or in any way related to my participation in this yoga class.



I acknowledge and accept I am participating in yoga at my own risk. My signature is binding to this liability waiver from this day forth. I have read this release and fully understand the terms. I execute the release voluntarily and with full knowledge of its significance and consequences.

 
             
Sacred Leadership Global Photography Release Form

I give my permission to Allison Adams | Sacred Leadership Global LLC and any media in attendance to publish any photographs or videos taken during online and in-person class to be published on any and all forms of media, worldwide.

I hereby agree that I will not bring or consent to others bringing a claim against Allison Adams | Sacred Leadership Global LLC on the grounds that anything contained in the property, or in the advertising and publicity used in connection herewith is defamatory, reflects adversely on me, violates any other rights whatsoever, including, without limitations, rights of privacy and publicity.

I hereby release Allison Adams | Sacred Leadership Global LLC., administration, employees, and volunteers and assigns from and against any and all claims, demands, causes and actions, suits, costs, expenses, liabilities, and damages whatsoever that I may hereafter have against Allison Adams | Sacred Leadership Global LLC.
